Prior to your first visit, please print out and complete health questionnaires from the list below, and bring them to your first visit. If you aren't sure which questionnaires to fill out, contact Jeanie. In addition, please bring a copy of your most recent physical exam, blood work and any relevant diagnostic studies from your primary-care physician.
